2/8/2024 EnchantaVaughn Approved QMHPA greeted Quaaniensha upon arrival to the home setting. Upon arrival to the home setting, Quaaniensha appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her day. She indicated to QMHPA that she was having a good day and that it was very nice outside and she hopes that it stays like that. QMHPA informed her the topic of today’s session. Quaaniensha did not present any symptoms of S/I or H/I at the time of the session. QMHPA began the session by inquiring about Quaaniensha’s current thoughts, feelings and emotions. QMHPA engaged Quaaniensha in a discussion surrounding her communication skills as it relates to her depressive and anxiety symptoms. QMHPA engaged Quaaniensha in a skills building activity. QMHPA explained to her that the purpose of this session was to assist her with being able to verbalize his thoughts through effective communication. QMHPA explained to Quaaniensha the importance of displaying eye contact, assertive listening and allowing people to know issues she may be facing. QMHPA educated Quaaniensha on positive self-talk and building self-esteem. QMHPA reviewed with Quaaniensha negative thoughts of herself and engaged Quaaniensha in a therapeutic activity on positive self-description. QMHPA conducted another skills building activity with Quaaniensha. QMHPA explained to her that the purpose of this activity was to allow her to identify positive traits about herself and those around her and eliminating negative thoughts. Quaaniensha appeared to be receptive to QMHPA’s interventions and services that were provided to her during the session. Quaaniensha displayed low self-esteem and is bashful. Quaaniensha said she wants to do better, be with her family, save more money and just be happy about things. Quaaniensha said she was friendly, a good person when she is doing good, nice, and polite. During the skills building activity, Quaaniensha was actively engaged in the therapeutic activity. She indicated to QMHPA that she does have problems with managing her depressive symptoms when situations happen.
